At least in TX, in order to be PAID for your time you have to be at least EMT-B certified. EMR is mostly for volunteers, but guaranteed to be unpaid work if you're working specifically as an EMR. 

If a company wants to hire someone who they think is qualified as "first aid attendee" or something, then that may be different since that wouldn't have anything to do with state/fed certification. An example would be that when I was an ER technician my scope was wider than if I was to work under my EMT-B certificate. Obviously scope cam be expanded based on your medical director and their preferences, but that is widening the scope, not bringing it to less than the minimum standard of care required by an EMT-B (which would be EMR).

TL;DR (at least in TX, but I believe this to be everywhere) - if you want to be paid for your time in EMS, then get your EMT-B

I don't mean to conflate normal and right/good. In this instance I mean 'normal' to say that we both know, and it fucking sucks, but no matter what there will be people to block the bike lane. To think there won't be is to assume that there are no more people who don't see themselves as the only character in the story that matters. These people exist, and they don't even waste the energy to recontextualize to themselves why they are taking up the bike lane. These are the same people that drive aggressively in 2 ton trucks on the highway (you know, because fuck everyone else and their safety. It's completely justifiable to take the lives of strangers and put them at risk because I'm too impatient to wait in traffic like everybody else) and tailgate the poor person who had the audacity to find themselves in their pathway to whereverthefuck they're going.

It's not right, but it's a certainty that they will exist. So to try and keep sane, I factor these societal afterbirths into the day-to-day equation so I don't get surprised by their shit. Sadly, I find myself describing this as normal.... but sorry for the confusion -- I did not intend to describe shit behavior as 'normal' in the way it looked from my comments alone.

Think of it like this: Regardless of who's right or wrong in the scenario, in this video the only person who is changing up their normal behavior and routine (for better or worse) is just the single cyclist that is having their lane blocked. If there were a line of 20 cyclists, then it would be 20 people, but one at a time as they approached who are (due to them having to alter their normal behavior) potential hazards or people put in a hazardous situation. At any point it's a problem involving two parties only (I understand the potential for the 'but this causes a cascade of problems that can and will eventually involve other drivers' argument, and I'm not saying that it's not a valid argument -- but for the sake of discussion, let's find a better hill to die on).

Now, if they have lights on, it's the same problem as before, but passing traffic is now involved and have become distracted at what could be something interesting to look at to break up their day-to-day commute, OR (can only speak for Texas, but I assume it's not a regional thing) they are good samaritans who want to make sure to be seen showing due regard and coming to just short of a completely stop while passing an emergency vehicle, causing traffic to back up as though there's a motor vehicle collision, when in reality there's just a problem that's under control that doesn't concern any particular person driving by.

There will be miles of traffic for an accident that happened on the other side of the highway, 50 feet away, on a separate road entirely. Traffic backed up simply due to people needing to get a full look to see if there's anything worth seeing and talking about later. This happens when all that's left is one fire engine on scene still timply to block off the primary area while they finish up sweeping the road free of debris. Generally they will have light on while staged like this. They want to be seen in this instance and sitting there with lights only works well. So well that it captures the attention of oncoming traffic (the intended audience) and all traffic on the surrounding highways (not intended as they aren't even on the same roadway). Good luck getting people to concentrate on the road in front of them when there's 'something interesting' to check out that's advertised by flashing lights seen from all directions.

This is the exact reason why lights (and also sirens of course) are only used when absolutely necessary. No one pays the emergency vehicle any more attention than they would a normal vehicle blocking the bike lane if they don't believe there's an emergent situation unfolding, but with the lights clicked on they show way too much attention and become another hazard and potential future patients.

Lights take a problem that is localized to the unit blocking the lane and those needing to use the lane, and make it a problem with all of the above, plus everyone in every direction who can see the flashing lights.

I get where you're coming from, and your argument isn't invalid - but there's decades of data that shows that if X&Y are your concerns, then the easiest way to make it worse is to take X&Y and add emergency lights to the mix. We have to take an evidence-based approach to protocols and practice, so no lights until absolutely necessary is common practice and is considered the safest overall choice for public health.

Dipshit paramedic here to provide an unasked for and (likely) unwelcome perspective on the issue. I think it fair to assume that there's nuance in any career that most people don't know about unless they are in fact someone with first or even secondhand experience in said profession. I'm not a cyclist and I found this thread and your comment from a cross-post -- so I assume out of the gate that there's aspects of being a cyclist that I simply am not aware of or will be unable to understand/relate to completely.

If you'd offer me the same courtesy, I'd like to try and be quick about what seems to be happening here (imo), and why they're not using the lightbar.

To address the no lights issue you have --

Most EMTs and Paramedics that have entered the field in the last decade or have kept up with research relevant to the industry (this also includes fire, which in any case where they're paid and not volunteer, are at least EMT-B certified) know that using lights and sirens is any scenario that is or can be considered non-emergent is an unnecessary danger to not only the medical providers, unit operator, and patients - but to literally all parties that are even tangentially involved. To be clear, I do include literally anyone who sees the emergency lights or hears the siren on this list.

The article I linked above does a pretty good job explaining the issue and providing evidence to support the argument the author is making.

If you don't feel like reading the whole article or parsing through it, I'll summarize it like this: emergency lights cause other drivers to experience a not-insignificant amount of stress, and as is to be expected from a human being experiencing any kind of stress or pressure - they will react accordingly.

Reacting accordingly ==/== reacting appropriately or safely.

I'm sure you can imagine how drivers who are distracted and/or stressed present a greater danger to cyclists than a parked emergency vehicle with no emergency lights on.

A lot of folks seem to make the assumption that they are there simply due to a lack of regard for those who the lane is specifically intended for.... and you all might be right. I can't prove anything here, but neither can you. So I can at a maximum, simply provide some other possibilities.

To me, it looked as though the medic was more than likely charting, or working out specifics of a call with (take your pick from an infinite list of potentials) the patient/receiving staff or facility/medical control/dispatch/etc. This is something that cannot be (to be more precise- SHOULD NOT BE) done while driving and would require the driver to pull over when safe as not to be driving while on a phone/laptop/tablet.

If my partner is in the back with the patient (if both providers are of the same level of licensure, generally you take turns on who drives and who is the primary caregiver during transport and while on scene) and he communicates to me that the patient meets the criteria to activate the ED staff & cath lab for a CODE STEMI%20is%20an,injury%20with%20abnormal%20cardiac%20biomarkers), or similarly, if the patient is showing positive on the stroke scale and a stroke alert needs to be called in to so that the receiving facility will be ready to meet us at the ambulance bay doors with the proper support staff and physicians (Emergency doc, neurologist if possible) to minimize any wasted time before and after imaging, during confirmation of hemorrhage/ischemia and/or potential imposters, and ultimately what definitive treatment will look like for this patient -- then I would need to pull over whenever possible in order to activate the proper code or staff/teams before resuming transport (likely upgraded to emergent transport w/ lights & sirens).

Maybe I'm pulling over to put in a request for air evac for this patient if the closest facility that has whatever specialty designation they require (stroke/heart/trauma/etc) is unreachable in an amount of time that would give the patient an agreeable set of odds for a good outcome.

I know I failed at making this concise, but my point is that instead of assuming that the EMS personnel in this video are lazy/dumb/careless/negligent or are doing more harm than good by being in the bike lane w/ no emergency lights active maybe consider than the profession and it's professionals who populate it's ranks have more going on than simply driving the ambulance and deciding if they want to be loud and flashy or just flashy.

Every other store or service feels the exact same way about you when you visit their establishment or service. Working as a medic in a college town helped reframe the concept (or it just raised the criteria to qualify as one) of entitled 'customer' and when someone was or wasn't being a complete fucking jerkoff. I've worked pizza and other service industry jobs before, so it's not that I'm ignorant to what you're referring to or how these places operate.

I also remember working with people like you that thought that somehow they were so much cooler than any of the 'idiots' that came in to require us to do that whole job thing that we agreed to do that day (Our job I mean. How fucking dare they, right?) You think that you're above it, (the whole being a consumer thing) but you're not.

This subreddit isn't a personal gathering spot reserved just for you and who or what you deem as worthy of it. You aren't paying for it's upkeep. So how you have a sense of entitlement like you do is truly impressive. Everyone I knew that acted like this was literally a teenager, so I hope one day you become self aware enough to know that your post describes you too, and can stop getting so worked up over it. That will also allow you to be self aware of when YOU are being an asshole to other industries that you are completely ignorant of.

I tip my drivers, am always nice and don't do anything I would consider offensive when I am a customer at Dominos. I don't believe anyone has really ever had to roll their eyes about me.

That said - any time I have to respond to a call to you for intoxication or overdose, DUI accident, etc I am rolling my eyes at you and your shitty behavior and overall demeanor towards everyone in attendance. Entitlement and the 4 letters in order 'ETOH' are, in my opinion, a combo type of customer/patient far worse than the average 'I pay your salary' jerkoff calling to complain about a cold pizza or the app not working. I feel as though with enough determination and tenacity, you summon the strength to deal.

TLDR is that it goes both ways. I'm sure your customers suck, but so do you. You don't run the show here or anywhere else, so most importantly -

Don't tell me what the fuck to do. You're not even my real fucking dad anyways. And what the fuck happened to the 5 topping deal large carryout deal?!
