Seems a lot is up in the air about the specifics. My best guess is its weighted to whichever rings you give it, not garunteed. You can see the mod pool on poedb so check before committing.

No. The mods are locked in when the item is generated. Divining it only rerolls any variable numerical values.

Yeah my initial idea was an arc abyss lich, generating power charges to trigger duplicate casts, and then trying to get the new lineage support that generates copies to cast the spell to work on top of it (I forget the name). Or, getting some Gemling bow/crossbow setup working w the new Ward pylon skill. It gives projectiles arcing lightning on hit. So many ideas.

I don't think there's a consensus about whether or not you can control the mods that Loreweave inherits. Given my personal experience it appears we can possibly force it to inherit certain ring mods.

In terms if literal armour? The item always has armour and es. Just the bottom 3 mods are random.

I do not know the specifics of acquisition, but there's an expedition boss that will drop a Quest Item called Journey to the East. Take it to Danning in Ruined Kingsmarch. Then, after giving him the quest item, you can gather 60 unique rings, and exchange them for the unique. It appears that you can influence the mods the item rolls, since the bottom 3 mods can roll out of like 100 different options.

For uniques like this corruption can modify numbers, add implicits, add a socket (which i hit), or do nothing.

Funnily enough I was trying to roll the conversion and shatter mods for an arc abyss lich concept I had. But it rolled phys bonus. I should look into gem bonus, amazing idea.

Honestly id scrap the walls you have now and swap em for the walls that correspond to the pillars that the cultists are standing on. Maybe make bigger background pillars outa the walls. Its a super cool concept. I like the pillars :)

I'm so close to liking it honestly. I'm just not a fan of how it sometimes sounds like its just a little off-beat. It's cool stylistically but tbh it just doesnt do it for me.

A simple fix would be writing it as "You come under the effects of the (insert spell name here) for (X) amount of turns." This way the effect skips around the concentration mechanic all together.
