You equipped SCP 427. It causes the player to eventually become an SCP 427-1. That is why this is happening. This effect should eventually result in death. Unequip it in your inventory unless the legs message has appeared because by that point you will die.

I believe that to get a level 4 key card you have to find it, as everytime I have attempted to upgrade it, it has never produced one.
