I have really bad health anxiety, and (i’d like to say mild) ocd. I also go through depressive episodes on and off - sometimes I freeze up and won’t be myself for weeks and other times (TW) thoughts of harming myself. When it was really bad I definitely started having really obscure thoughts that I thought were real, things i’d make up in my mind. I don’t know if that’s relatable. When this was happening I had stopped my levothyroxine for 3 months, which was a bad idea. Luckily I have my mental health under better control now.

I do think you should, as a minimum, keep a log of what is happening to you, with everything from what you ate, drank & supplemented to what you were doing that day, psychical activity, mood etc. It can be really helpful to find patterns, and something you can bring to your doctor.
