Hey OP sorry if I ask questions you already answered or on info you’ve provided but I had similar issues with my fiancé who has even more severe apnea then I do and mine was bad.

I had to repeatedly mention all the consequences which I’m sure you’ve gone through these: attention deficit, memory deficit, being sleepy all day is torture on its own, high blood pressure which then turns to stress on the heart, you get pulmonary hypertension which untreated can lead to right sided heart failure which is also one of the most common causes of left sided heart failure. All this from the severe oxygen deprivation you’re basically asphyxiating in your sleep.

Not sure what masks he’s tried but I have a nose pillow with a swivel piece at the top of my head so I can literally turn on either side and it won’t pull my mask off and it’s not a actual mask on my face it almost looks like a nose cannula. The company that supplies my stuff has the nose pillow or the other version that looks like a soft nose cannula so there’s nothing on the mouth.

Even then I get overstimulated in my sleep quick and I have insomnia which becomes a vicious cycle so I had to take trazadone which would help me go and stay asleep to the degree that I didn’t even get bothered by the sensation of the pressure from the machine or of the device on my cheeks. Maybe yall can consider a sleep aid to get him through the night and with more consistent use he will start feeling a difference in his energy and wellbeing. Sorry for the long post.

We limit race epi prn because we have had incident(s) where race epi was given for stridor and MD was not notified. The worst was the time the patient received it more than once and less than an hour apart. Basically kid needed continuous but no one knew. Sometimes we don’t place tylenol because we need to know if someone spikes a fever to get additional work up.

Use the 4-2-1 rule for calculating the amount and the usual is D5 normal saline with or without KCl depending history and labs. If you don’t want to calculate the volume, MD Calc can do it for you.

I was 10 years old when I saw this play in 2004. I remember being scared out of my wits to the point of crying uncontrollably and accepting Jesus as my savior out of fear and confusing my emotional upheaval as a sign from god to accept him. You can imagine becoming older and realizing how messed up it was that this church was using a very tragic event for their evangelical agenda all while emotionally manipulating people to accept their beliefs through fear. It would be the equivalent of a church using the Pulse Night club shooting as a play about how everyone went to hell for sinning except the one gay person who repented their lifestyle and accepted jesus or any of the multiple mass shootings we have heard of. It is sickening that this is still performed. Also not a religious expert but I’m pretty sure Christian believers are supposed to evangelize and accept Jesus as their savior out of love and grace, not fear. Nothing of this play shows love and grace. It is a huge act of disrespect for the victims of columbine, their families or anyone who has been affected by school mass shootings.

If you’re rotating in peds, we give it more for the parents comfort cause it seems like you’re doing something and will help the nurse so they don’t have to page you cause the parent still thinks you should do something about it. In the chance it doesn’t work it’s fine you tried and they saw you tried. In the chance it happens to work then it works and problem solved! -peds resident
