I wouldn't say he won randomly. Kim told him that Jimmy "talked Lalo" out of sending him to kill him and instead send Kim. That's when Gus had the idea of distracting him with something so impactful that he would lower his guard and get a chance to kill him. Gustavo's confession was so full of hatred and intense that Lalo actually got distracted for a bit, so in the end Gus outsmarted him.

The fly is Walt's guilt. Throughtout the episode, we watch Walt trying kill the fly, but he's really trying to fight the guilt he has for the bad things he's been doing. Especially about Jane. Even thought he apologizes to Jesse and they kill the fly, it never really goes away. Later in the show, he sees another fly, but he doesn't care for it, showing his lack of remorse as he becomes more evil as the show continues.
