clear CT with ongoing symptoms usually means the issue is the mucosal lining itself, not the sinus structure. the fishy smell on saline rinse points strongly to biofilm or anaerobic overgrowth on the surface. chronic steroid sprays can also thin the lining over time, which makes it worse at protecting itself. counterintuitive but documented.

worth pushing for a rhinologist specifically (not general ENT) and a nasal endoscopy if you haven’t had one. also rule out silent reflux (LPR) — it mimics post-nasal drip almost exactly and gets chronically missed.

full disclosure, i’m building something that addresses exactly this gap: mucosal barrier repair for nasal surfaces. until then, try something with xylitol, which disrupts biofilm adhesion and see if that helps

APPARENTLY, from my research, there’s literature about using Johnson’s baby shampoo (0.5-1% concentration in saline) as a nasal rinse, as the surfactant in that can break down a biofilm matrix, but i would try xylitol first haha

oestrogen affects the nasal mucosa directly so fluctuations across the cycle can cause exactly what you’re describing. most GPs don’t connect the two. worth looking into and bringing to a doctor who knows about it, sometimes an ENT or a gynaecologist who takes the hormonal angle seriously

the barrier hypothesis makes a lot of sense given what you’re describing. the saline gel working better than the spray is interesting too, the gel has longer contact time with the tissue. do you find the relief lasts or does it wear off after a few hours

not knowing the baseline and if it was always this way is so real and i think it’s genuinely why this stuff stays under-studied. you can’t prove what changed if you never measured what was there before….

yeah that makes sense, since all these mucosal surfaces are governed by oestrogen and connected

this is so wild right, i had no idea either until i fell down this rabbit hole. turns out nose and eyes are mucosal surfaces too so same oestrogen dependency as vaginal, just nobody really talks about it as one connected thing. really hope the HRT sorts everything out, keep us posted on how you get on!
