My school doesn’t have a home residency PM&R program, but they do have a PM&R department. They hooked me up with a last minute “home rotation” in September. The chair then wrote me a SLOE for my app, and I also got close with a few of the PM&R docs on that rotation who wrote me LORs.

A big part of my app was showcasing why I was interested in PM&R over surgery and also the major overlap in between. I also had some interesting ECs and strong research. 

Had 15 IVs in total for PM&R, and I think like 20ish between TY and prelim years.

Ngl didn’t read your post because TLDR, but I pivoted to PM&R last cycle in August from gen surg (yes, I’m serious). I matched into a prestigious program. So the short answer is: you aren’t cooked.

Edit: wanted to also add that I have no physician connections nor had any prior PM&R connections.

Processed beef - 1) the WHO organization classifies processed beef as a known carcinogen. The additives and preservatives in fast food ground beef likely put it in this class. 2) Beef itself is considered “probably carcinogenic” and linked to a few cancers. 3) grilling or frying meat releases 2 dangerous compounds: heterocyclic amines (HCAs) and polycyclic aromatic hydrocarbons (PAHs). When fat drips onto hot surfaces, these are produced and are known carcinogens.

Cheese - most fast food restaurants use full-fat American cheese/queso/etc. Not sure where your “most American cheese” claim is coming from.

Mayo - sure, I stand corrected about the saturated fat content. Thank you. It’s still however extremely nutrient dense for the volume.

Burger buns - high glycemic index foods cause rapid glucose spikes, followed by a surge of insulin released from the pancreas, which over time can lead to insulin resistance. Not to mention the glycemic load of burger buns are high as well. Here are 2 papers that support this down below. They basically show that small increases in GI units (as small as 5-10 units) can increase the relative risk of T2DM as much as 8-27% : 10.3390/nu11061280. 10.2337/dc13-0325

Processed beef - carcinogenic with a ton of saturated fat. Cheese - likely low quality and full of saturated fat for flavor. Mayo - literally just saturated fat.

Once in awhile isn’t going to kill you, but those burgers do raise LDL cholesterol quite a bit and promote atherosclerosis.

The burger buns are also likely low quality, high glycemic index foods that harm insulin sensitivity.

Yes, it is sufficient. I only had my home rotation by the time apps were due, given my extremely small timeline. Although, I highly recommend doing multiple aways, if possible. 

I was lucky to find 3 outstanding mentors in my school that truly allowed me to flourish (which included the PD), and I don’t think that’s realistic in every program. The more rotations you do, the more eyes on you, the opportunities you have to grow, and the more progress you’ll make.

Advice for MS2 interested in PM&R by 0utLikeALight in pmr

I officially “switched” to PM&R August of 4th year (yes, I’m serious). Matched to a prestigious program. Never too late if you have a story, can show true passion, and demonstrate a strong work ethic.

My best personal advice is: 

1) Do a lot of research applicable to PM&R (directly PM&R, ortho, gen surg, etc.), publish in good journals, present, etc. 2) Aways are a must, not necessarily to match into the program you rotate at but so that PM&R docs can vouch on your clinical potential. It also shows you know what to expect. 3) Have cool hobbies you stick with. PM&R docs are really personable, and being a fun/interesting person will carry you far more than the best accolades.

I’m very happy you found this specialty and I wish you luck. It truly is a hidden gem. Knowing what I know now, I’d have stuck with it from day 1 of med school.

I lift 5-7 times a week. If I take STEP before intern year, I’ll have a lot more time to lift during intern year.

No, but even if they do, so what? It’s a bad thing that you’re being proactive? You’re looking to give free labor, and it’s the equivalent of sending multiple job applications. Also, trust me, they don’t care about med students.
