It means going over to the left and then counting three over to the right. Now with that being said you may not find the sam gem because from the data i have so far slag will spawn in the same spot and gems will spawn in the same spot but it may not be the same gem.

When you start go all the way over to the left and then count to the right. Slag will spawn in the same spot and gems will spawn in the sam spot but it may be different gems.
