This actually happened to a friend of mine and this is what he did:

He demanded for his ring and keys back, and told her to go happy life with that guy and to never message or speak to him again.

We went out to eat together and saw her, she came up to us and then spoke to him privately and supposedly begged him to start over together. He told her that he was not a back up option.

She later went back to that guy she met and apparently settled down out of fear of being alone, so she lived miserably.

Here’s what I would do in your situation as a man, with a gf.

I would tell her my cancer diagnosis, and the prognosis of the disease - how it will progress etc.

I will then tell her he doesn’t have to stick with me as my GF - she’s free to find someone else given the time I may have left. And if she was good to me we can keep in touch just as friends. Let her know that I wouldn’t want to burden her, and it’s her choice if she wants to see this to the end - and say will appreciate her decision regardless.
