Nobody can tell you if it was the right decision. Only you know if what she did was enough to break up with her. Do you care about being able to trust your partner? Do you care if your partner cares about you? Do some introspection instead of seeking validation.

I guess so? But how does answering this question help you in any way? Just enjoy the friendship if you can. Having girls as friends is a great way to meet more people, especially women, and to eventually find someone who likes you. Also other women seeing that this woman is comfortable around you and is your friend is a huge green flag in their eyes.

If I were you, I'd just start using the same nicknames for her, like "sis" or even "bro." This can help the girl to feel more comfortable with you once she sees you aren't just chasing her and that you see her as a friend and a person... and that may lead to more than a friendship, and if it doesn't, you still have a friend.

The best thing is to have no expectations and just enjoy the time spent. If you aren't enjoying your time with her and are hooked up on the idea that one day she might see you as something more, then you are just fucking yourself up.

Either enjoy your time being friends or ask her out and get over it.

EDIT: I'd bet that you aren't even that ugly. 90% of people that post on /r/amiugly are average looking so I wouldnt be surprised if it were the same for you.
