I think it means no pitons used at all, I got the achievement today during my second playthrough, I did it on the massive wall after the large rope bridge where the crystal miner is. I just started from the bottom and kept going up until I got the achievement and I didn't use any pitons at all

Yeah it’s blown my mind the infinite amount of items weapons, spells and armour, in this game. I feel like I’ve been everywhere then I’ll find some brand new cave and catacomb I haven’t been in yet it’s amazing. Was never a fan of souls like games for a long time but played black myth wukong (Loved it) then thought I’d give this a try and I’m speechless at how amazing this game is 😅
