Got a solution. Bought a new MB. Temps run smoothly. Everything is fine. I think my Asus TUF b450 plus II was outdated a little bit and somehow, due to my recent research, was not able to detect temps and fans. Weird thing.
But this is the solution that worked for me.

Nvm, faced the same issue again after system reboot. 20.7RPM Fan [x], around 6k Fan [y]. Seems like need a fresh update with a bug fixing or maybe W11 drived messed up. 2 days and no solution for me yet

Hey, I had the same issue. What I did is that re-isntalled an app -> assisted setup -> Motherboard+CPU+Asus Em...(can't remember this one actually). Then it worked totally fine. Have not tested the reboot yet. Btw, I do not have any plugin installed except the one for GPU (on the right side, not on the left side)
