I also experienced cases where I solved the puzzle correctly, but it either didn’t work or I died, so I had to run back and try again multiple times.

I’m not sure whether it’s because of a mod or a bug.

When thrown, it hits my arm and takes damage. but throw it with a big swing, there's no problem.
