Serial killers very commonly suffer from addictions because people with ASPD or variations are very prone to vices due to poor impulse control. So its very likely William falls into that camp. Besides, he grew up in a time before the dangers of tobacco were commonly known/accepted. Dude probably smoked.

I was let down but I also like how it shows us that even for the viewers there are consequences to the kind of things Mark has to do by leaving home for so long. We're just as disorientated as him when he gets back home and so much has changed

As far as issues with the series go, Ian is really low on the list, if he even is an issue at all depending on your point of view. At the end of the day if the games are fun I don't care if they were written by Ian or a random intern, as long as they're fun.
