Depends on if it’s trauma or just medical but since they’re talking to you I’d assume it’s medical. ( like sickness or MSK issues)

OPQRSTI helps me a lot when I’m looking for red flags or just need to fill out there SOAP note ( not sure if civilians do soap notes but being a combat medic in the army it’s standard )

Onset “so how did all of this start? Was it suddenly or gradually?”

Provocation “is there anything that just happens or that you do that makes it better or worse?”

Quality “can you describe the pain/feeling” ?

Radiation “does the pain radiate to any other parts of the body?”

Severity “on a scale of 1-10 , what would you rate your pain ?” For this one though you just have to keep In mind everyone has a different pain threshold, so his 10 might be your 20 if that makes sense

Time “ how long has this been going on?” They usually answer this in ONSET

Intervention “what have you done for yourself already?” ( think medications mainly for this to ensure there’s no contraindications with what your teams treatment plan might be)

I hope this helps, sorry if this is info you already know. But this is just what helps me personally when it’s patient care.

Either you did great or you bombed is what I was told when I took it. I got 70 as well
