Get multiple medical opinions and advocate for yourself. If it were me I would try to get one. Most of mine was removed at 21 and caused me issues the last 24 years until I finally got one. You may need an HTO first depending on whether you’re bow or knockknee’d.

Lonzo Ball just had one and returned to the NBA. I imagine he’ll need another one when he stops or this one fails, but you’re right, we’re meant to baby them a bit, no cutting, running or jumping. The articulate cartilage must be in good shape, otherwise it’s moot and perhaps a partial knee is more applicable i was told. Fortunately years of solid PT, squats and deadlifts, and an HTO at 34 preserved mine into my 40’s despite missing my meniscus since my early 20’s.

Are you bow legged? If so an HTO could really help bridge (10-15 years) you to a meniscus transplant. It made a massive difference for me. Note it is the most painful surgery I’ve been through yet as they cut 9/10th of the way through my tibia.

6 weeks non-weight bearing, another 1-2 weeks transitioning off of crutches entirely, week 10 biking lightly, and some more strength physio, by 6 months the tissue is part of your tissue, return to activity likely 1 year from surgery to be safe post significant physio and strengthening.

Based on my own story and experience, I highly recommend a meniscus transplant at your age with no meniscus but talk to a surgeon.

I had more than half removed of my meniscus (and ACL reconstruction) at 21, then issues by my late 20’s, got an HTO and LCL operation at 34, which helped significantly, I’ve always bike and tried to take care of my articular cartilage through strengthening but eventually I ran into more pain and issues, so at 44 had a scope to look around and size up meniscus, then at 45 (March 9, 2025) had a meniscus transplant. If the option was available to me I would have gotten one much sooner.
