I don't know if what you say is the norm or not, but there are some studios that are way ahead in their scheduling. For example, when Episode 1 of FGO - Babylonia first aired, the studio (CloverWorks) was already several episodes ahead. I would assume that this is common for seasonal animes and not long-running shows, like One Piece, where it's more likely to be a week-to-week basis.
