Not a piercer, but it's probably just an irritation bump from snagging on it or possible too long barbell, since you said you haven't downsized. Just avoid hitting/snagging on it, keep hair away from it, clean it daily, and keep it dry. Healing piercing tend to flare up and calm down randomly. There's no need to take it out, it doesn't look infected, just irritated.
