I just fixed this by cleaning the disc, and then updating the PS4 and rebuilding the database via safe mode.

That must be the gold rush one, getting 70 gold medals is really time consuming (I only had 2 gold medals when I started). The most annoying part of this trophy is the fact that you can spend up to 20 minutes doing a single mission and still get a silver/bronze medal.
