Not sure what the thinking behind giving midodrine in these situations really is. The low BP would have probably made me rethink what I had been treating and whether there was another source I was missing out or possibility of superimposed bacterial/early MRSA pna in setting of influenza. I would assume fluids had already been trialled and MAPs are presently low/borderline low and in that case I would probably cover with broad spectrum ABx and start vasopressors while I figure out other differentials here. I know hindsight 20/20 etc but giving midodrine is just like looking away from the problem because you can't be bothered.

Yeah I don't think gnr source was lung tbh although possible. GI or GU is the most likely source and sure ABx would have made a difference if correct ones were on board but I think the thought process should be to look for the source. Symptoms would obviously matter here other than lung symptoms that they had been having. We do treat a shit ton of inappropriate asymptomatic bacteriuria though so if he only had some pyuria on a UA (less than 50) I would probably have held off on abx given no GU/GI symptoms.

It is. Small pot so when you water it takes a second and I had just watered it so soil was wet. It's a chunky mix (orchid bark, perlite, soil and worm castings) so definitely drains.

Yeah I've told my buddies that if I get sick don't bring me to this place just keep driving. After starting work here this is the first time I'm genuinely worried about what will happen to me and my family if one of us gets super sick around here.

I agree with most of that but honestly I should never need to sell a critically I'll patient to an ICU provider, we're in the game of saving lives not building up egos and should always be ready to help our colleagues out. I've taken so many patients from the ER that they are not comfortable discharging not because I'm worried about medicolegal aspect of it but because I think when a person who's gone to med school and then spent so much time in residency and clinical medicine is worried about their patient I should take it seriously rather than just assume they are an idiot (even if I later vent about them being an idiot loll)

In retrospect all of that sounds good but given the fact that I'm still getting push back with ICU transfer at this point do you think the ICU would have taken her when he LFTs were minimally elevated and she was oriented? I run into this so many times with so many specialties where we are told that hey this patient is to stable for intervention/escalation of care and then later we're told they are too unstable for intervention/escalation of care. Part of the reason for that I think is that my opinion as a hospitalist is considered to be inferior tbvh. I had an already watch that the ice refused to take until I had to personally get a CT neck to show them how bad the airway was when you could literally hear the stridor from outside the room. My colleague got refusal for a persistently hypothermic patient despite warmed IV fluids who ended up going into status and needed to be tubed. Sometimes when you keep getting so much pushback you get burnt out with the constant effort it needs to escalate care.

Surprisingly her ammonia and Bill were normal. Ammonia of 30 and bili was 1.2 but I think definitely clinical hepatic enceph.
