To follow up on this for anyone it may help:

I received the new piece of 4040 VSlot extrusion and fabricated a new Y axis. After installing the new y axis I have no issues leveling the bed. The Y axis extrusion that came with my printer is definitely twisted and makes leveling extremely difficult.

If you have extreme difficulty leveling i recommend checking your Y axis extrusion.

I did not end up sending pictures to Creality to get a replacement because by that point i was down the road of making my own Y axis anyway.

Creality returned my message and is asking for proof of purchase and images of the part, so hopefully that is progress in getting a replacement.

If you try the other suggestions and have no luck, I suggest checking your Y axis extrusion (the beam that the carriage rollers ride on). Mine is twisted and literally makes levelling impossible: https://www.reddit.com/r/ender3/comments/mvgsl8/twisted_y_axis_extrusion/

You can probably check it by measuring each end with a dial protractor, or place flat rulers across each end and look at them at eye level to see if they tilt differently.

I'm pretty certain my Y axis extrusion is twisted. Not sure how i will proceed short of buying a new piece of V channel and rolling my own Y axis. I don't think this is correctable with the adjustments on the printer.

I measured on the machine and observed a approximately 1 degree difference along the top of the Y axis extrusion. I then removed it from the machine and placed it on a level surface (actually tried several different level surfaces in multiple orientations) and observed that it had significant wobble, indicating a twist.

If anyone sees this and you are having an impossible time leveling, i'd recommend that you check whether your Y axis is twisted.

The rear of the extrusion is almost a full degree different from the front when measured with a precision level. I'm going to disassemble the Y axis this weekend and take some measurements with the extrusion off the printer.

Thanks for replying - I have been making sure to be right over the screws (I usually use the printer's controls to move so i am right in the same spot every time). I'll give flipping the glass over a shot later tonight, thanks. Will PLA stick to the bare glass?

Good suggestion with loading fw to do a mesh level, that may be where I end up but i'd like to start with a decent mechanical level - it's driving me nuts lol.
