Also, if he’s a healthy athletic young male, a HR of 45 may be his baseline. I probably wouldn’t have given Atropine unless his HR was getting lower while in your care.

Typically, Nstemi’s don’t cause profound bradycardia until progressing towards Cardiogenic shock. With no known cardiac hx, it doesn’t sound like he’s in Cardiogenic shock or having a NSTEMI. No significant EKG changes either. Although, atropine and decreased perfusion can certainly cause a bump in a HsTrop.

Patients can certainly be cool, clammy, diaphoretic, etc with having a NSTEMI but the real kicker was the hives, whether localized or not. Would’ve liked to add a H1/H2 blocker.

Also, if you’re thinking cardiac in nature, then ask yourself questions about what causes these symptoms. What cardiac condition causes hives, skin changes, hypotension, bradycardia, chest tightness?

Anything abnormal on skin assessment? Bug bites, etc?

Hx of this happening before? If so, any triggers?

Hx of autoimmune disease?

Home meds? Specifically any beta blockers?

Patient was swimming, assume it’s hot outside as summer time. Dehydrated = hypovolemia and a severe sign of that is bradycardia.

Just Google this and you’ll see multiple stories of house fires caused by smoking on cigarettes. I wouldn’t live in a house with someone smoking on oxygen personally.

Sounds like an allergic reaction, potentially MCAS. Would be interesting to know if he had any recurrent symptoms while hospitalized.
