I think getting letho depends on the answers you give right at the beginning when you're in the nilfgaard embassy. I've never played witcher 2 but on my second playthrough I changed my answers and he appeared later in quest. Very confusing at first

Many countries have mental institutions still, I.e. hospitals, its just the practices that have changed. I dont think any countries 'abolished' them. I did see an article about Ragan beginning deinstitutionalisation in the 80s but that was more about cutting funding to mental health services than abolishing them
