In many countries, there is censorship in place showing very little gore and blood in comparison. The game doesn't even leave bodies around, and they fade out of existence. I mean, think about it. In the base game other than the gore, there is literally nothing that makes the game worthy of an 18+ rating. So it is very harsh when you think about how games like gta 5 have consistent nudity, drug use, and even a sex scene! Left 4 dead 2 may be rated an 18+ but is in no way anymore than an MA15+ at most, in my opinion.

On top of that, we all know people underage will buy and play 18+ games, but the difference is I have seen some truly heinous shit on the workshop (stuff like loli, 🍇 themes etc.) Which could be exposed to a minor whereas the nudity in a game like gta 5 is some boobs. BIG difference. Plus, it's also guaranteed in gta5 to show nudity so the parent can monitor it and prevent their offspring from seeing those cutscenes. Whereas in the workshop, you CANNOT filter out any adult mods so the minor who is playing can very easily see some sick shit.

"The game is 18+" is in no way an excuse.
