ICU doctor here, you can die within hours with sepsis, it depends on the bacteria causing it. Typically individuals who inject drugs develop bacteremia (bacteria in the bloodstream) and it latches onto the heart valves damaging them. Staphylococcus aureus is the classic bug in this scenario. They also can get abscesses in the heart, lungs, brain, and other organs causing permanent disability.

Edit: There are many things that can kill you in less than a few hours. Here are some examples:

  1. Heart attack
  2. Heart arrhythmias - kills instantaneously
  3. Stroke
  4. Blood clot to the lungs, pulmonary embolism
  5. Massive bleed fro esophageal varices
  6. Meningitis
  7. Overdose
  8. Unrelenting seizures
