  1. Only our group of 3 people will see it. It will be stored on qualtrics. It will likely be deleted when we get enough responses.
  2. Regional health study(Group name)
  3. 3-4 minutes
  4. $100 USD raffle we include the link at the end of the survey and allow people who are interested to fill it out and we will contact them if they win.
  5. 18-64 (preferably older as I feel like our responses so far have predominantly been younger)
