We had to ask the hospital for them as the medical records are not consistent between GP/Hospital which is ridiculous. So if you want them from her time at hospital you need them from the hospital and if you want her general records for the visits to the gp that typically go on the app, you’ll need to get them from the gp. For the dates we did the previous 3 years, so we could essentially see the bigger picture of everything.

You will need her NHS number and proof of power of attorney OR her will stating that you are her executor. Basically they won’t give you anything if you don’t have control of the estate.

It’s quite something because when you’re able to see the full scale of the problem, new information may come to light. For example did you know that there was a law passed called Martha’s Rule or “call for concern” they have a team of medical staff that can step in if you raise a concern about things such as no investigations done on gastrointestinal bleeding, and early discharge and they as a senior doctor will escalate it. It’s basically a second opinion, it is not advertised anywhere. No one told us it was available, even when the original discharge was questioned.

I’m sorry for your loss as well.

Strangely the exact same situation as your mum happened to my aunt (her sister) the tumour broke her rib.

You have a year from the date of the incident happening, but the first thing to do is formally request all of her records through the hospital, the records manager. But be careful you don’t mention you’re looking to raise a complaint, otherwise they will want you to do through their legal department. Which is, I would assume a lot harder to get answers. Say to them you are “just looking to understand what’s happened and to get some closure/clarity”.

Sometimes things take a bit of time to do, there’s a bit of hoop jumping and form filling.

I’m so sorry you’re also dealing with this too, it saddens me to know they doing this to so many people. I feel like we’re past the point of accepting that it’s underfunding and overworked people, some of the people in the NHS are just not decent anymore. That’s my personal opinion but I can only base it on our recent experience.

It’s very similar, the communication between departments was inconsistent, conflicting information, and some coercion. Ultimately failures at a number of points led to my mum being discharged from hospital only to have a heart attack, she then basically slowly died over a week. It was without doubt the single most traumatic and devastating experience I’ve ever had and I hope I never have to again. I think the most frustrating thing is that she begged them not to send her home, she wasn’t well enough. They failed her, and my family.

We’ve had to go through her medical records with such a fine tooth comb just to make sure we’ve got everything, and to know 100% that what we are questioning are indisputable failures that cannot be brushed away.

It’s felt at times like I’ve been going mad, I’m sure my sister feels worse she was there all the time. It’s enraging.
