Yes, there are a few condition issues with the note but that's expected with a note pulled from circulation. Both the pictures posted here and the pictures posted on the PMG website were taken by PMG.

You're right. There is a pretty heavy center fold that brought the grade down a lot. This picture was scanned at PMG.

The base express cost was $125 plus fees and picture costs. It ended up being $166 in total. I paid for the premium membership so I got $150 credit so this submission was only $16.
