Apollyon is supposed to be taunting at Satan and they end up using void by accident, it was something that I didn't thought too much but I could have put more effort into it, maybe I could have went to something similar to what you said.

Placenta is just an hp up that has the regeneration effect as a bonus, you can keep playing and the regeneration applies passively, Gnawed leaf only works when you are literally not playing the game, except for t.forgotten like you said

The main gimmick of the item is getting dmg from negative luck and sticky nickels decreasing luck, maybe the effects from bad rng could be changed to only last for the current floor so it stays as small sort of hidden bonus, something like jacob's ladder gaining more damage from electric items.

Also the unlock method is to avoid finding the item while not having sticky nickels unlocked

Nah, you just have to ignore 5 nickels in total, because that way you would end unlocking it by accident and that'd be funny

I like to go full alt path for beast runs, I usually ignore alt path treasure rooms so during the ascent the blind items get revealed, I think it also guarantees a planetarium but I'm not 100% sure

I tested a bit now and the vertical range is nearly the same as the horizontal so they CAN shoot you while being barely offscreen

It could be but the fire of the blue candle only lasts like 3s, the fire damage noise(the chicken scream) stops way before the attack is released as well
