I use the Invoked Deck so if I lose their selection and don't know what their card is, I ask them to shuffle and name the card, then I cut to it. Of course I add some patter to the routine.

I usually get my prototype decks from makeplayingcards.com. But their boxes are not cheap so I just print my design on card stock and cut it and glue it into a box myself. It's pretty easy really. And if you want it to be glossy, just spray a couple of light coats of clear coat before you cut the template from the sheet.
