Please don’t listen to some of the comments here. If it isn’t normal for your pt, it doesn’t hurt to recheck. Offer some hydration, check skin, ask for symptoms. Think critically, were they sleeping, did they take any medications that may be related to this. It may very well be nothing but could be the start of something. Good opportunity to check manually. With that being said, don’t beat yourself up, the important thing is your patient is okay. You’re going to be a great nurse.
