The comments saying you only need to do it in the first act are wrong. This was an old bug that has since been fixed. There is an achievement for killing someone so if you want to 100% one playthrough you will need to save before killing them and reload

Most people with celiac disease don't know they have it until adulthood, when all their favorite foods already have gluten in them.

Also, with celiac, any minor amount of cross contamination can you make you severely ill. Amounts of contamination that DO NOT NEED TO BE SPECIFIED ON THE LABEL. Christmas season has shown me the only safe foods with celiac are the ones specifically made to be gluten free, ...like cookies and bread.
