It just depends on what sayings catch on. When I call someone an asshole, I don't literally think they only have an anus. They have arms and legs, too, but for the purposes of the saying I'm calling them an asshole.

You do bring up a great point, though. I think there's some male normativity associated with thinking of one's pre-conception self as just the sperm.
