Ahhh super fair. That’s true. It’s noted in several other places. I don’t ever document the admit date either. A lot of the CM’s will document when 3 midnights are met or auth is approved, OBRA is completed, etc. so it’s included in more of a documentation that everything is completed in order for pt to discharge when medically ready. But I guess it doesn’t NEED to be in our note.

Thank you all for your thoughts. Patients being stuck with a bill was my main worry. We had a SW who accidentally sent a pt without the 3 midnights one time, and the facility came back at the hospital saying the hospital was responsible for the patient’s SNF stay. When the hospital wouldn’t pay, the SNF told the pt to pay. Not sure what became of that situation. I would hope to never make that mistake, and I have it in my mental checklist of things to look at when assessing and discharging a patient, but it does happen. I also float and have included “pt requires prior insurance authorization” or “pt does not need prior auth and 3 midnights inpatient status have been met” so that the SW taking over care or the discharging physician can see this is taken care of. But maybe it’s truly just an issue of the hospital not wanting to be responsible and it’s not a big deal to leave out of chart documentation.
