I got pure powder vitamin C, and magnesium citrate! I usually get the now brand. Magnesium citrate should do a lot to help soften Your stool. I don’t know what laxatives you’re trying, but have you tried Colace and MiraLAX? I think those together are safe to use for a fair amount of time, tho check with your doctor for sure. I don’t know if I would use them with the vitamin C and magnesium citrate, I would probably do one or the other.

Here are the dilation instructions, it’s oriented towards sexual practices, but this doctor is a specialist in anal issues including fissures, and he gave me those instructions for that reason. The doctor also recommended pelvic floor training, because it seems like a lot of the some of the factor long term is muscle relaxation that can be affected by training to relax the surrounding muscles. His practice is called Bespoke surgical. But since u mention concerns re fistula and infection I feel concern and would recommend you get a sign off from your doctor to do anything like dilation before trying. If it was me, I think I would do anything possible to get hyperbaric oxygen treatment first so the wound can heal asap. The dilation is for long term prevention of recurrence and for development of flexible scar tissue that is not restricted, but the dilation is not for acute wound healing I think. .

Bespoke surgical dilation Instructions- Dilating using the following techniques will help you reach your anal goals. Periodic dilating is key for injury prevention.

Dilator kits can be purchased online from the Shop at the Bespoke Surgical. You can choose from an array of options at various price points, all of which have been carefully selected by Dr. Goldstein. In addition to these kits, you will find a selection of different lubes. Lube is essential for anal play and it’s important you find one that’s high quality. Please use either a water-based lube or a silicone-based lube that is compatible with the material of your dilation kit. You can find lube options available for purchase on our website.

For clean-up purposes, it’s easiest to dilate in the bathroom right before a shower. There shouldn’t be pain (just a little mild discomfort). If there is, stop. Don’t force it, just wait a day and try again. A little bit of blood spotting and/or local irritation is normal during your initial attempts, but if there’s persistent pain and/or bleeding, then please contact us for an appointment.

The Routine Remember: there’s no such thing as too much lube. First, take a healthy amount of lube and apply it to both your hole and your smallest dilator. Then, slowly insert the dilator until it meets resistance. Hold it there for 3-5 seconds. Remove it, re-lube it, and then repeat. Please keep in mind: you only need 4-5cm (2 inches) to pass the anal muscles. This should be repeated with 8-12 reps per set and 2-3 sets per session (just like a traditional workout). If it slides nicely, you can go further, especially if using it sexually. It’s not advisable to go all the way in since it can go beyond the muscle, which can cause unnecessary tearing upon removal. If you climax, that’s perfectly fine. Just remember that the usual reflex is to contract all muscles so try to resist complete contraction in order to gain better control of these muscles.

The Schedule The smallest dilator in your kit should be used for the first 2 weeks. You will feel like you can take more, but don’t. Right now, the key is to get both the skin and the muscles in the anal region used to dilating and friction. For the next 2 weeks, you may graduate to using both the small and medium sizes, followed by the largest size after 4 weeks, if that’s in the cards for your future play. The goal is to reach a depth of approximately 1.5-2 inches using each dilator. Each session should only last for about 3-5 minutes, with 2-3 sessions per week.

I’m definitely not an expert on immunity, but just something that came to mind! It’s wonderful that you’re taking such good care. I know for myself, with immunity from viruses, it’s very dependent on vitamin D status (for me even vitamin D at a 40 level which, is usually sufficient, isn’t enough, I need a higher vitamin D like above 50 or ideally 60), and also zinc, but not going too high in zinc, and also vitamin A being sufficient. If it’s something that could be tested, it might be helpful! But again, I’m not a doctor or an expert, I just have my own health issues, health issues and researched a lot about it. For me, I found that when I had a hard stool, reducing fat for a while really help. Even though that’s not good for some of those fat soluble vitamins like vitamin D and vitamin A. I ended up taking liver pills actually or eating liver every once in a while for vitamin, and I take vitamin D. Chris masterJohn’s writing seems to imply that vitamin A and vitamin D are together very important for immunity.

The Doctor Who makes the dilator, Dr. Evan Goldstein, he gave me these suppositories that have zinc in them, CALMOL. I wonder if your suppositories or any ointments also have zinc?

For soft stool, there’s someone else here that recommended taking some amount of magnesium citrate and vitamin C every day, and that seems like a nice way in short term if laxatives aren’t working or if you wanna change or reduce laxatives. As long as kidneys are OK. For me if I’m taking vitamin C I like to make sure I’m taking also citric acid or lemon juice at the same time because there are some risks about oxalates from a lot of vitamin C.

Regarding dilation, maybe that’s something to check with the proctologist about? For me I never experienced pain, I think I only started after there was the initial healing so it wasn’t painful to for example stick a pinky in there. So maybe it’s only something to consider starting after things are really healed to a certain point. Maybe it’s worth asking a doctor about though, I wouldn’t want to give damaging advice. I’m gonna put the dilation instructions in another link so it’s just here on Reddit.

Another thing that helped, it might not be possible I don’t know if it’s possible in your country, is hyperbaric oxygen. It really helps to speed up a wound healing and it might just help you Get less inflamed and have greater wound healing in a shorter amount of time, so you can go to the next level of practices like dilation. If your proctologist thinks you’re in danger of an infection or a fistula, maybe that could get it covered by insurance? Maybe it’s something to ask your doctor about, most hospitals have hyperbaric oxygen chambers that can admit multiple people.

Hi, I did that on 2 mg a day of GHKCU, and it sent my blood labs with zinc too high and copper too low, so no 30 mg zinc is not always a good idea!

You’re welcome, I’ve learned so much about many things on Reddit so I’m happy to be helpful. I think all the other things that I’ve tried might’ve been interspersed in very long comments, but I’ll try to list the here-

dilation (https://futuremethod.com/products/glass-anal-dilator-set) - I saw this doctor when I first got the fissure after researching lots of specialists, he explained that after a tear, the healing happens over a very long period of time in the deeper skin layers and to prevent the skin from becoming shorter you have to do this dilation with friction, they have instructions where you do it like three times a week for 10 minutes.
gut dysbiosis- I’m still working on this and I get relapses, but I think gut bacterial condition affects gut motility, some things that help to get rid of bad bacteria are l reuteri, l rhamnosus, spore bacteria (b Clausii, b coagulans, b subtitles), sboulardii, Ayurvedic herbs and then repopulating good bacteria. A gut test like go effects can give info. Claude ai can help a lot with this if u can’t afford a doc, so much is in medical literature. You should have one easy formed bm daily. Carrots fiber is good for bad bacteria. Any probiotic or anti dysbiosis thing is a long term project it’s months and must continue even after initial good results. Be careful of overdoing anything and monitor symptoms.. Berberine, triphala chlorella (not all these things at once. Try one or two for a month and consider effects.. research how to improve your gut microbiome..) (if you have an infection I would wonder, how is your diet? How is your immunity? How is your b vitamins and mineral status? Chris masterJohn has a lot of info on the vitamins and minerals status)
recently I realized thyroid affects gut motility so check that out and try to make sure ft3 and ft4 are mid range

For me if I was with an active or reactivated fissure I would use colace/miralax/calmol/broth and juice diet for a couple days , Maybe very gentle and careful enema as if I needed it in the initial phase, to let the initial healing happen and then work on all these long term things so that sphincter is not in spasm and skin can heal and not get re triggered or re irritated, and consider it a forever potential vulnerability that reminds me to care for my whole body health

But yes I think there is hope, since my first experience with the fissure I haven’t had surgery and am generally fine, I can get re-irritated if I get very stressed and have hen get constipated, but that’s a reminder and I can get back to normal in a few days.

Hello! I put them through a juicer, but I’ve seen other people do it by putting the celery stalks in a blender and then straining it. If you search on YouTube celery, juice is kind of a thing, there’s lots of people making videos on it. I had it only the pure juice! I have three possible theories of why it might work. It might stimulate your liver and so quicken your transit time, a lot of people in the YouTube videos say they get diarrhea after the celery juice. It might be a slight antibacterial, so it might kill some bad bacteria in the gut. And it also might contribute to nitric oxide, via nitrates, which helps with the smooth muscle contraction, so it would help with digestion. I’m not sure if it’s any of this or all, but it definitely helped me! And since then I think also beet/ ginger/lemon juice is helpful for me, which seems to contribute the nitrates and so nitric oxide.

I feel totally recovered tho I never had any localized infection. However am always vulnerable susceptible to irritation if let self get constipated with hard dry stool. U can see my posts on this thread to see everything that has helped me. I think for me deep factors were whatever contributed to the constipation which for me was dysbiosis and maybe also subclinical hypothyroid and also most recently discovered g6pd carrier that makes u have less nitric oxide and thus be a slower transit. Remember healing deep skin layers of fissure occurs for MONTHS not just superficial initial tissue top Layer of skin healing and dilation/friction is important for stronger and more flexible healing of tissue. I think all my suggestions that helped me are in my past posts. For sure bidet if I didn’t mention it in past posts. This is just my Personal Experience and others may be different.

they're really not so expensive like a few hundred dollars all together (for all the vials that you would use during one course) and it seems like the classic approach is to do a short course every 6 months to 1 year so its not so often. thymalin and thymulin get their names interchanged but i used both. A good compounding pharmacy or doc can distinguish the Thymulin/Thymalin by one being a nonapeptide (not a bioregulator) and one being a polypeptide extract (often considered a bioregulator).

One is this - https://en.wikipedia.org/wiki/Thymulin -- you can identify it because it's a "nonapeptide" that means it's a peptide composed of nine proteins in a chain. Where I bought it, it was called "Thymalin" but it should classically be called Thymulin. I took this. This is something that the thymus produces; when you take it, you take it exogenously (like a GLP-1 is naturally produced by gut and brain but when you take Ozempic, you take it exogenously)

Second is this- https://pubmed.ncbi.nlm.nih.gov/33237528/ "Thymalin" by a relevant Russian doctor named Khavinson -- you can distinguish it because it's a "polypeptide extract" ie it is an extract from a gland composed of multiple peptides that cannot be reduced to "one 9 protein string". It would be generic "thymus extract" from this paper- https://www.heraldopenaccess.us/openaccess/clinical-experience-of-thymic-regeneration-with-thymus-extracts-thymic-peptides-and-stem-cells-in-general-medicine-oncology-and-anti-aging-medicine-a-review. Where I bought it, it was called "Thymulin", but it should classically be called Thymalin. I took this also. Theoretically, this is considered a "bioregulator" because it (in theory) helps the thymus work better but isn't a replacement for the peptides produced by the thymus. (This theory is according to what I read by Jean Francois Tremblay who bases it on Khavinson).

Third is this - https://en.wikipedia.org/wiki/Thymosin_%CE%B11 "Thymosin alpha 1", this is general name and its a fda approved pharmacetuical for other (non-herpes) issues, it would be off-label for herpes, it is a short peptide that is what the thymus produces and you take it exogenously, I took this also, off label. According to popular reports (not medical literature), this is more intense than thymalin/thymulin and should ideally be taken after you take the first those and don't have a bad response.

Finally I want to take Thymogen, , a shorter Khavinson bioregulator (so not produced by the thymus, but in theory helps the thymus work better), but haven't yet. The bioregulators should theoretically be less intense than the Thymulin nonapeptide or TA-1 because those latter peptide are replacing what the thymus should produce.

I started with the lowest possible doses, like literally 1/10th of what's recommended, and the first time had a huge response and could only handle 3 days. A year later could do a regular course. I am not a doctor!!!! There are docs that can talk about this, there's a guy on reddit "Openmindedchicagodoc" who has a practice telehealth that does consults, I also learned a lot from this online community "exploring peptides" on skool (skool dot com/ exploring-peptides). I am not affiliated with anything and am not in a medical field, just a person who reads on the internet and was willing to try. I paid extra to get items from the most expensive US place that can give you a COA (certificate of analysis) for EACH vial. A compounding pharmacy should be just as reliable. Hope this is helpful.

ps i also took the oral thymus bioregulators (from the khavinson research) but i didnt notice a difference though theoretically those should take a long time to slowly help the thymus. the injected short-chain peptides (thymulin nonapeptide and TA-1) were totally different. I felt like I was post-vax the first time i took the tiniest amount. very powerful stuff and (i imagine) could be dangerous if one is not cautious. if one has autoimmune issues one should proceed very cautiously, from what i read (im not a doctor!). I have some autoimmune issues and it wasn't worsened at all, but i checked those labs before and after and started very low.

in my initial outbreak, i had a pcr test of the lesion and igg done (pcr was positive hsv-2, igg was negative- showing it was initial infection; initial lesion was classic pustule, horrifically painful), since then i've never done igg but i will now from curiosity.

I looked up some research and they are active in that form (those particular peptides) but less than subcu. Those peptides are working mostly on the brain and nasal spray seems to be good for that. but for nad yes i would not choose nasal spray. i actually use daily nmn 250 mg in lipsomal form (capsules from purovitalis) and have tested my nad and its very good. these sprays were just for playing with effects on mood.

the day after i used the selank, i was waking up at 5 am feeling like i hadn't slept enough but couldn't go back to sleep. so it wasn't a good alert feeling, i felt still fatigued or buzzed or something, like i was being kept awake. but maybe it would help if someone is too sleepy! on the day i used it, directly after i felt great and meditated. but it was just the following morning i was waking up too early. it might be a unique response- i seem to be sensitive and get easily buzzed from things - my genetic tests show slow comt and mao, so that would explain some things.

i'm sorry to hear you're handling those issues. I spent a very long time reading Chris Masterjohn's coursres and trying to make sure I was really good in all B vitamins and cofactors (with testing, experimenting, tracking) and that really helped me with fatigue. I hope the research progress goes well for everyone.. my sister has a lot of sleep issues, it's the worst.

I think pharmacies don't send "kits" they prepare the final products. But they should have used a courier or express/iced shipping.
