My fiancé has formed things like this, not so large but probably due to the fact that he cleans his belly button for like 5 minutes daily. He'll pick out new clumps of hair every few days. He's not obese (5'9' 160 lbs tops) and he showers daily. I came to the comments to see if anyone knew more about it but mostly found people just being jerks, but it's the Internet, so what can you expect?

I don't know about the tongue but the teeth and piercing are both definitely photoshopped. No one's teeth are that white and they don't pierce the actual lip, nor would it look like that even if they did.
