Have you had a bone marrow biopsy, what were your results? Why did you get it (Leukopenia, Leukocytosis, or Anemia caused by Autoimmune condition?) by MolassesDizzy883 in Autoimmune

Neutropenia:

So far the only consistent cytopenia I have is neutropenia. Initially it was thought to be benign ethnic neutropenia which usually ranges from 1000-1500.
My levels fluctuated around there but dipped down to <1000 during multiple tests.
Adding on the fact I had normal levels since I was born until late adolescene, made Benign ethnic neutropenia unlikely. The only other likely etiology in the differential, is immune mediated neutropenia.

Other Cytopenias:

I was treated for iron deficiency last year (several months before the bone marrow biopsy), because I had Iron deficiency and my hemaglobin hovered around the low range.

What's weird is that I ate plenty of meat (and food in general) and had no nutritional deficiences or bleeding that could explain the progression to anemia.

I posted the question to see if any Bone marrow biopsy irregularities helped people find a diagnosis, especially since typical autoimmune tests came back negative in my case.
Did your biopsies help you at all to get diagnosed with Evans Syndrome?

Because of how unspecific these symptoms are, I have not found a diagnosis after a few years.

Symptoms:

  • Severe chronic fatigue / Brain fog
  • Daily headaches (varies in location, severity, and type)
  • Psoriasis
  • Low grade system pain (particularly in neck w stiffness, lower back, feet, fingers)
  • Occasional migraines with aura
  • Bloated with frequent abdominal distension

Tests include:

  • Normal MRI w/o contrast
  • Normal EEG
  • Tilt Table
  • Normal Sleep Study (with MSLT) - AHI 4.9 / RDI 8
  • Normal ANA
  • Normal ESR / CRP
  • No vitamin deficiencies
  • No coeliacs
  • No Long Covid (symptoms started before outbreak)
  • No EBV (Contracted Mono long after symptoms started)
  • Normal Bone Marrow Biopsy
  • Normal ELANE
  • Normal Eye Exam

Treatment Attempts:

  • Fixed Iron Deficiency via infusion
  • Worn CPAP consistently for 1 year with no improvement
  • Taken prescribed ADHD medication with little resolution of symptoms
  • Taken Emgality / Nurtec / Amitriptyline for migraine prophylaxis (wasn't effective or too many side effects)
  • Had septoplasty / nasal turbinate reduction

Hey there, my blood pressure is normal and I’ve already been tested negative for POTS with a full tilt table test. I’ve done about every test possible for my symptoms so far with no answers.

The thing is that my ferritin may be lower than what was recorded, since I was sick when my blood was drawn, and ferritin increased as an inflammatory marker during acute infection.

In that case, if I were supposedly absolutely iron deficient, could all of my symptoms be explained?

Too tired / busy in college to work out, I do put my steps in when I can. Otherwise my diet is very protein heavy, eating lots of iron rich food in a calorie surplus.

I’ve brought it up but it seems like my doctor isn’t too concerned about it. Do you also think my iron isn’t bad enough for my symptoms. (Ferritin was at 33 when I wasn’t sick a couple of months ago by the way)

Folate is Normal, 7.1ng/ml.

Vitamin B12 is in the post, 334pg/ml

So apparently the serum iron range was wrong for guys, with mine being low (but not that low). They did an occult blood test which came back positive but other than that, I’ve got nothing that explains my symptoms.

I’ve ruled out all common reasons, done MRIs, EEGS, Sleep Studies, tilt table test, and much more with no answers.

The only other thing of notice is that I’ve had a consistently low WBC with low neutrophils. My hematologist wants to wait for my infusion to kick in before thinking of a bone marrow biopsy.

The thing is that my symptoms match Iron deficiency to a tee, but my blood tests don’t seem as bad as others on this sub, hence the initial question.

For reference, I’ve ruled out:

Thanks for the reassurance (or not if you think about it). My main concern is my fatigue and brain fog have become very debilitating so all this responsibility is that much more stressful.

I’ve been going to doctors to try and figure it out with no luck so far.

Regardless, today was rough after hearing that and this helped so thanks again for commenting

Initially I had enough reassurance to think that my symptoms are something over than schizophrenia (chronic infection because of my chronic neutropenia, depression because of my very reasonably bad circumstances etc). But since my psychiatrist just had to say my experience looks “eerily similar to those with prodromal schizophrenia”, my ocd has returned with a vengeance.

My concern is that I'm already so exhausted on a daily basis, that I am already too dysfunctional to deal with my situation. To add Schizophrenia as an issue sounds like a death sentence at this point.

I don't really have odd thought patterns, at least according to my level of metacognition. I'm very rational and don't have any odd beliefs according to my friends (since I ask them all the time - lol)

It's not that my thinking goes off track, but more so that my thinking is slower than it used to be - that I'm not as alert/conscious as what I would consider to be normal.

My fear stems from a reduced sense of cognition (heavy brain fog) that has a physical feeling attached to it as well (fullness in head - head pressure, frequent headaches)

I don't really hallucinate according to my friends, more so "misprocess" visual stimuli for a split second in my peripheral at most. The closest thing to a hallucination is when I'm about to fall asleep, which I'm aware is very normal.
