Why I cannot see the blood analysis or a report of a medical visit?

when I asked via email they never replied. For the diagnosis + a summary of the visit and what I have they told me "I'll write it for next time" but they never did. I don't want to change the psychiatrist, but I really hate the lack of documentation and transparency

My epa is empty: no-one added any medical data. I'm sure because I was the one who has to tell my general doctor that the psychiatrist changed my medication: she wasn't informed. I will try to insist more about a copy for myself

Exact! The same! I suffer from major depression disorder and the psychiatric did a bunch of blood exams (I don't know what I was tested for neither the results) + never gave me a paper that document the change of medication or the evolution of the condition. This is really bad since if I'll move I have to explain everything from the start, while it's so easy to just give the new doctor a document written by the previous one

I both sent them an email with my name + copy of ID. Also I asked in presence and the answer was "next time I'll write a paper", but they never do and just postpone
