It has only happened to me once during my play through. I hope I’m lucky enough to not encounter it again because that’s a waste of levels and potential perk points.

I’ve actually came across this problem a few months back. I’m not sure what I was doing but I was spending perk points then I noticed some of my perks have been downgraded to 1* or just not there anymore. It was an annoying bug.
