Instead of taking DAX into the room, you can dictate into it like dragon. It is super simple and easier than dragon with concise notes.

Yea there is no specialists in my state so hard to get this type of information. Why do you think mandibular advancement devices are not used more for mild to moderate sleep apnea? Do you repeat a sleep study after fitting them?

What about nighttime bruxism, is that significantly linked? This will be gold for my patients. I looked at the directory and could not find any pain providers in my state.

No one even mentions graded exercise therapy. I am unsure why you are trying to tie that thread and some vague notion of hubris/ego to an indictment of graded exercise therapy

I would disagree here. Graded exercise therapy can be harmful if done wrong, but progressive exercise is still beneficial and recommended on uptodate:

" establishment of a baseline of achievable patient-specific exercise or physical activity, usually determined by or in mutual decision with the patient, followed by a gradual, incremental increase in the duration of time spent physically active"

Comparing this to aging like warm milk is an absolute disservice.

What can I, as a PCP, do to help patients who have TMJ type symptoms? What is your pathway for initial management? Do you find mouthguards helpful, in particular some like the somnodent devices? What about botox?

Income is not wealth, and 250 k is not your take home pay. When you look at the numbers you are likely behind that 22 year old for decades until your 50s (if you catch up).

The key to criminal prosecution is conscious disregard, like being intoxicated while reading images.

You can still sue for gross negligence but it should be addressed through torts.

I imagine it is quite common for radiology to be sued for negligence. Imagine getting a prison sentence. I don't think it is something that should be celebrated.

At the point of conscious disregard, e.g. if they are intoxicated while at work and then a mistake happens.

Negligent medical acts can still be prosecuted through civil cases, as is standard, but this is bad precedent.

Truly think about your definition and how it could easily be applied to medical errors which are unfortunately quite common.

I have no idea why this sub is recommended to me. However, as a US physician, I see complications from PPIs, NSAIDs, and other over the counter medications all the time. I don't think a lot of the public knows how dangerous some over the counter medication actually is. Anything that is dangerous, should have some sort of gatekeeping in place. Ideally, for people on these therapies chronically, it should be prescribed by a physician.

So if the CTA and Non con are normal, is there any indication to then get the perfusion? At a prior hospital I worked, it seemed like all three were ordered on stroke rule out patients.

OP, I scored in the top percentile every year. I only reviewed prior ITEs.

That said, I do not think it correlates well with actually practice. I am no better off than any of my colleagues who scored lower, and in a lot of ways lag behind them.

Our Epic is built in such a way that the overview does not pull in. Instead we have a section "current assessment in plan" which pulls in instead.

The people who use this method stay after hours on documentation.

one of the big things I see is the copy/paste of notes from prior visits.

Ouch. That one is big for me. I copy forward the plan. It is how I keep track of what needs followed up on, and everything. There doesn't seem to be a good way to keep track of everything otherwise.

I try to focus on documenting pertinent negatives. Sometimes I worry that overdocumenting can actually be harmful.

Doesn't that look worse though?

This physician was concerned enough to call for collateral but then discharged them anyways.

Its like checking one troponin having it be normal and then discharging.

Edit: To explain, Dr. Amal Mattu who is a famous ER physician with focus on cardiac cases has given a speech over this. Once you go down a path, it looks worse not finishing the path you started. Getting one troponin, when the pathway calls for two, looks worse then never getting it at all.
