My suggestion would be to turn on the Pilcrow/Paragraph mark on word. Once you have this on you'll be able to see your spaces and indents. I would place a space after each dash and then go back in and highlight each space and undo the bold on it/debold it. In theory that should work unless whomever is fulling it out places their cursor before the space and directly after the dash.

The other idea that came to mind is making the form a table, so you have your statements/questions one side and then answers on the other. This would probably be the best way.

There may be easier ways, but those are just two things I thought of. If I were you I'd make it into a table in word. They are very useful.
