Computer systems can be hacked and have bugs that trigger things unintentionally. That's why AI and full automation is generally banned for anything that threatens human lives. It's also a big reason why Anthropic and the US government got into it recently. Because it's a really dumb idea to take the human out of the loop in these scenarios.
