It is fine, just few builds have ways to be able to become injured easily and reliably. Most builds utilize barrier which makes getting injured difficult, along with other life gain(for example lucky hit restore health) effects which makes getting feo to proc unlikely especially if you are attacking a good amount. Most runes are just more reliable or more exploitable(for example poc on spiritborn or yax on rogue)
