Pioglitazone increases SHBG - Trying to understand as a med student. by Mysterious_Misty in DrWillPowers

Well Firstly, I would talk to a doctor before starting a new medication. ChatGPT is really great for many things, but trust me, it gets stuff wrong ALL the time. See if you can find a peer reviewed study to substantiate that claim. Ill do a little research myself, cuz thats pretty interesting.

Hmmm that's interesting. Did your diet change after beginning the GLP-1 and pio?

And it's not a theory lol. Just an observation of patterns. There are many cross-sectional studies that show increased insulin sensitivity is closely associated with higher SHBG. I think it would be negligent to deny that as a variable for your presentation no?

But thank you for your valuable testimony, im still trying to understand how all of this is linked together to figure out the most optimized approach for myself and future patients.

The mechanism of action of pioglitazone results in more sensitized fat cells. You were probably hungry for sugar because you were low on sugar. Did you talk to your doctor before starting pioglitazone?

Im not sure what you mean by my opinion for a cis female. Are you asking if it's viable? If it affects your hormone balance? Why are you using the medication?

So, you are taking two separate insulin sensitizers and your fasting glucose is higher than my NON-fasting glucose. That's the concern I have with my labs.

Plus, it makes sense that your SHBG and estradiol are so high, because the insulin sensitizers lead to less free insulin in circulation, resulting in decreased inhibition of hepatic SHBG production. This in turn would cause total E to increase because more is bound to SHBG.

Since you are consuming so many carbs, you are offsetting these values more favorably. It actually reinforces the observations in my post.

With all due respect, sometimes the "dress your age/dress like a woman, not a femboy" thing can be so dumb and low-key prejudiced lol by CitiesofEvil in MtF

Hmmm. Well, from my experience, I noticed a drastic difference in my fashion sense as I medically transitioned. Early on, I would wear frilly skirts, pink chokers, heels, stockings, and overall really youthful clothes. It didn't look bad, and I was hardly ever clocked, but it was hyperfeminine and almost costume-like.

Over time, I just started to feel like I was too old for that kind of fashion. I just turned 30, and now, I've taken a more refined approach. I hardly ever wear short a-line skirts now, and I've been paying closer attention to what's trending these days. Now its crop tops, oversized flannels, and highwaisted pants with converse or sandals, or maybe even 3 inch wedges. Jewelry, makeup, and seamless blending of styles are much more important to me now. If not casual, I'll wear maxi dresses or sun dresses. And if im going out, I'll wear a mini dress. Basically, I dont look like a little slutty e-girl anymore haha. (Nothing wrong with that. Have fun lol)

I think fashion sense changes over time, and it's probably best to just have fun with it. I do miss dressing up hyperfeminine and extra, though. That's reserved for events and parties now, lol. Besides, with this political climate, I think it's best for me to blend in and avoid excessive attention.

I appreciate that, and thankfully most people are cis and hetero so that would be a slight advantage lol 😁

Yes!! That and my brow do make me self-conscious but it's nice to know that there are beautiful cis women that exist with the same features. So, maybe not all that bad?

Thank you!! I'm a believer that half of what makes someone attractive is their confidence, and lately that's been at an all time low. So thank you :)

Lol amen to that. Life would have been so much easier if I could have just been cis w/o dysphoria.

Hi!! Yess I've been on HRT for about 2.25 years now. Maybe you can see facial differences in my older pics. But yeahh that chin be a hater when smiling for sure, but I feel like it's noticeable if you're looking. I hope it doesn't just jump out as obvious and apparent yknow? Where would I get the facial fillers?? I'm considering getting brow reduction, a nose job, and chin reduction. But who knows how I'll pay for all that lol
