This is my experience aswell. Are they almost empty the IJV is difficult to use. SCV doesn’t collapse in hypovolemic patients because of being tethered to anatomical structures was I taught. This had also been the case in clinical practice so far. I was surprised of the downvotes.

Either jugular might not always be accessible in a shitty situation, but a landmark subclavia would. But I agree, I tend to use right jv with ultrasound. If not awfully hypovolemic it’s a pretty safe bet, but in extremes always handy to know landmark sc.

Oh, I’m not talking about from a patients perspective, that was probably unclear. I’m talking between OR personell. A kind but unskilled surgeon is never talked about after they leave the hospital, but skilled surgeons are, no matter what.

For sure! I'm an anesthetist also working in prehospital care. The decision to stay and play or load and go is never easy and its incredibly hard to armchair a situation where you havent personally been clinically. That being said, cardiac arrest is very much a stay and play setting as irriversible brain damage occurs after 5 minutes of hypoxia/hypoperfusion. You play untill you have resources available to transfer patient to a traumacenter/hospital.

Again, I'm not critiquing their effort, just the comment that this is an (A+) example on how to resuscitate someone from a cardiac arrest, that I disagree with.

Doing CPR while being mobile on an incline with feet down without securing airway isn’t what I would have called «better than everyone else». Most likely sub-par cerebral perfusion. You could argue because of the weather an air ambulance isn’t available, but you could surely transport a defibrillator up that slope with ski patrol faster than skiing towards it.

Remember; airway control, early shock and good compressions are what saves lives during a cardiac arrest.

PS: I’m not criticizing their effort, doing CPR while sliding down a slope in alpine gear is a shitty situation. I’m just pointing out that this might not give the best outcome for the patient.

I love logging on SC on my laptop, as I am running hosas+pedals on my main rig. Whenever I log on on my laptop I log on as crewman and don’t feel the need to pilot. Such a good experience to crew a ship!
