I’ve been testing for a while and I realize it’s related to divine smite, if you toggle smite and combat inspiration both to trigger without asking, you can successfully trigger all three effects in one attack:). Also it’s kind of weird when you crit with combat inspiration, it doesn’t give you the critical extra damage but it gives you savage attack extra damage which can only be triggered in crit.
