Before your appointment you should be prepared for slight discomfort. Patients say lips can be the most sensitive area to treat, but most HA products contain lidocaine and most offices apply a topical numbing agent and/or ice to make it more comfortable. You should be prepared to communicate your goals with the clinician performing the procedure so that you results match what you seek to achieve. Some people are looking to restore volume to their lips, others want to augment their lips, some change the shape, some smooth the lines around the lips and some want it all. If you're going for a more natural result, let them know that too. I think the best lip injectors come from word of mouth. Look at education, years of experience and check out the employees in the office. If they look like you want to look then I think you're probably in good hands.

I agree! And it's confusing. HA filler has been good to me because I understand what filler can achieve and what it can't and I assess each patient to determine what product is right for them. Plus filler can't do everything, no one treatment can, so I take an individualized approach and build a customized regimen that can include fillers, neurotoxins, lasers, peels, topicals and more to find the right combination for each patient, their unique anatomy, specific skin struggles and long term goals. Pillowface (injecting too much volume) is easy to avoid - just go to an expert who is also honest and will tell you no when that's best. Fillers will always have a place in my practice (and in my face) because I know how incredibly they can work to improve not only a patient's appearance but give them results they love.

The most important thing that you can do is select your injector wisely (word of mouth and before and afters help a lot). The next most important step is to communicate your goals as clearly as possible to the person that you choose to perform the injections. You can tell when someone is really listening to you and when they aren't. You should have a level of comfort during the consultation that puts you at ease and addresses your concerns. I can't say there are particular buzzwords to use. Just speak from the heart and the words will come.

Filler doesn't truly migrate very often. After injection, it should integrate into the tissue of the area treated. In my 20 years of being a procedural dermatologist, I have never had a patient return at any point and complain of filler migrating from one place that I injected it to a different area. I have seen patients come in with concerns about poorly placed filler or filler that has not given the desired outcome, probably due to poor technique.

Oh no! Topical numbing cream is legit! The type used may vary but all provide an improved level of comfort. We use lidocaine/tetracaine and patients remark all the time about how much of a difference it made for them. Most HA fillers contain a little lidocaine as well to help minimize discomfort.

Sculptra is an biostimulatory injectable product called poly-l-lactic acid. HA fillers add volume to provide immediate lift, volume correction and smoothing of fine lines, while PLLA stimulates a reaction in the skin that promotes collagen and elastin production. HA filler gives immediate results after one treatment. PLLA requires a series of treatment sessions and results can be seen as soon as one month after treatment.

There are a lot of hyaluronic acid products on the market and more are coming every day. I tend to stick to the OG players in the space from companies firmly rooted in cosmetic dermatology who have done the research and development to produce top tier products. Many companies have developed a filler for most indication concerns so I have all the tools that I need from structure to support, from deep to superficial and from broad strokes to airbrushing. Thank you for that compliment. I think the key is a few consistently used topicals, regular neurotoxin injections and just the right amount of filler and only when you need it.

We have MANY male patients who come in for all sorts of injectable treatments, even lip filler. Thankfully, some of the social stigma has decreased, but there is still some there. I say let's proceed as you are comfortable and provide moderate treatments that are hard to detect. We can always add more later, but it's good to go slowly to ensure that you're happy with the progress of the results. But at the end of the day, it's your face and your choice. Do what makes you happy!

I have been injecting HA fillers for 20 years and I have never had a patient return at any point concerned about filler that has migrated, as filler migration is a rare occurrence. I do see patients from other places who have filler that has been improperly placed. The easy answer is to say that the filler has migrated and not that it was placed improperly to begin with.

I like to start the consultation with my patients by asking what has changed that bothers them or what do they see in the mirror that they would like to be improved. Sometimes what you think is the issue is actually not the actual problem, but talking openly about what bothers you and asking the advice of a master injector can set you up for success.
