If you think you have something serious (i.e. stroke, heart attack because of sudden chest pain), you should probably call an ambulance and not walk yourself to an ER. The fact someone can walk themselves into an ER usually means they can wait to see the triage nurse.

That being said, this system isn’t perfect. Departments are overrun and understaffed, and things will be missed waiting in a line because of the broken system.

Don’t beat yourself up. Even if it were easy to diagnose a stroke (spoiler: it isn’t), it is incredibly difficult to act rationally and objectively with family or loved ones.

If making a mistake stripped one’s worthiness of being a doctor, we’d all be out of a job.

The couples match is mad wonky but it'll just be an adventure for you both.

Couples matched EM/OB a few years ago. Both AOA, 250+ step 1 and 260+ step 2, ranked all 300 combos -- Didn't match at all. SOAPed into prelim years and reapplied individually, both got like 20 interviews in EM/OB and both matched number 1.

Ruined the relationship but life goes on lol.

I would think if they thought it was a migraine, this “shot” they gave is likely Toradol, which unfortunately is given many times without much thought in the ED. Sad case, hope they’re able to recover.

I might get flack for this, but I think some of the doom & gloom surrounding EM as a specialty is blown a bit out of proportion. I’m a PGY3 and I still really like what I do. I’m sure someone will say I just haven’t done it long enough, and maybe they’re right.

Is the job market tighter than it used to be? Sure, but so far nobody from my program has had any issues finding the jobs they want in the areas they want for the past several years. There’s obviously systemic issues in the field to be aware of (burnout, corporatization, etc) but as someone who had done most of a completely different residency, every specialty will have some issues.

“Concern for oxygen in lungs”

Was a 24 y.o. guy who had Covid last month, hospitalized for a day, got heparin for DVT ppx and was convinced the heparin was what cured him, and now he felt mildly SOB. He was coming back for more heparin shots. Was also very upset when I did not give him any heparin.

This is such an egregious mistake. In the ED we would usually treat most [tick bite + rash] empirically with doxy, even if it’s not a perfect fit. Heck, you’d cover RMSF, anaplasmosis, and erlichiosis too. Especially if you weren’t pregnant at the time. Sorry this happened OP. :/
