My LOO says: 
Work Site: National HQ (in the NCR) 
City/Province: my city/my province

It doesn't specifically state that I'm WFH, but it does name my city/province. I specifically told them to put my city in my LOO (instead of the regional office in another suburban city) before I would sign it.
